Hangout: USNA Induction Day / I-Day Class of 2028: Thursday 27 June 2024

I'm having trouble logging into the ABS with my normal email. How were you able to do it?

Clear cache. Try different browser. Assume some updates/maintenance might be going on over the weekend.
What is ABS? Is that something an incoming plebe needs to know? My DS accepted but hasn’t received anything other than a few doc links to submit in his portal.

Instructions are included. The 1st paragraph describes the assessment.

Same here! Came in an email. The online links show the same info.
DS confirmed the same this morning. We are wondering if this email is the formal “permit to report” or if there is an official letter (via email) to follow. Regardless he has started on all the requirements/ documentation. Exciting times indeed!

The ABS questionnaire is not loaded yet. There are a slew of 2028 parents on the USNA BLB 2028 Facebook page. We are all helping each other out with questions and answers. Many of us have done this all before. You might want to join the page. It's very helpful. Start going through the PTR b/c it take awhile to complete it all. For example the Security Clearance "cheat sheet" the plebe needs to work through is over a 100 pages. The plebes need to submit information for their clearance and this "cheat sheet" they will bring in with them so they can have info they may not normally know (for example your neighbor's address and phone number from a house you lived in 5 years ago, or a passport #, work numbers etc.). Anyway, all the links are available on the PTR page within USNA.edu. Slowly go through it...checking off completed forms as you go. Have two folders going, one for forms to bring on IDay and one for forms you will mail for a May 15th USNA delivery.

The DD 3111 needs to be sign off by your doc or an immunization clinic
I know that the immunization record form must be completed by medical professional......does the DD Form 3111: Routine Immunization Screening Form also need to be completed by medical professional or just the individual and its brought with on I-day......
Plebe candidate fills that out and brings to I-Day (one mailed)

Where did you read this? Does my doctor need to sign the "Form reviewed by" part?
Where it states form completed by and reviewed by. When the plebe goes to get his/her Tuberculosis shot/prick/blood drawn have them sign off on it. Parents can sign off on it but we had the clinic sign off on the DD 3111, the Tuberculosis paperwork and the full immunization form.

DD Form 3111 can be completed by the parent or candidate. On I-Day the Naval Health Clinic will review this form. (in case if you need any vaccines on I-Day)

"DD Form 3111 is a crucial tool for healthcare professionals to assess an individual's eligibility for routine immunizations. It helps ensure that vaccinations are administered safely and effectively, taking into account the individual's medical history and specific health conditions. By carefully considering the responses to these questions, healthcare providers can make informed decisions about the timing and types of vaccines suitable for each individual."

Where did you get: "(for example your neighbor's address and phone number from a house you lived in 5 years ago"?

"List the places where you have lived beginning with your present residence and working back 10 years. Residences for the entire period must be accounted
for without breaks. Indicate the actual physical location of your residence, not a Post Office box or a permanent residence when you were not physically
located there. If you split your time between one or more residences during a time period, you must list all residences. Do not list residence before your 18th
birthday unless to provide a minimum of 2 years residence history.
You are not required to list temporary locations of less than 90 days that did not serve as your permanent or mailing address.
For any address in the last 3 years, provide a person who knew you at that address, and who preferably still lives in that area. Do not list people who knew
you for residences completely outside this 3-year period, and do not list your spouse, cohabitant or other relatives as the verifier for periods of residence."
