[speaking in reference to USAFA P-School]
In order: Not exactly, Yes, and 1 year.
You can apply directly for the P-School or you could be offered it when denied from direct appointment (my case). It isn't a 100% chance that upon graduation you'll go to the academy, but unless you do some major mess up, you can safely say you will attend upon graduation.

I just read from the USMAPS FAQ page:
"It is important to know that all candidates must apply to West Point to be considered for USMAPS and that candidates may not apply directly to the Prep School."

Sorry realizing now there is a difference between USMAPS and USAFAPS. I thought USMAPS was just an umbrella term for all of them. Disregard my earlier statements. ^^
