Security Clearance question

I'm doing last-minute review on my DS security clearance form and I noticed that he did not put in his dormitory address in the 'WHERE YOU HAVE LIVED' section. He has stayed in a university dormitory for the last two years and has gone home once or twice (weekends) per month during the school year and during school breaks. Should his dormitory be included in the form ?

He needs to show the physical address where he lived for the last two years.
