It appears in reading the Vision Survey form that even if you do not wear glasses or contacts and you passed your Dodmerb vision screening that you are still required to receive an eye exam from an eye physician or optometrist. Is this correct? I also assume that this cannot be completed by your pediatrician.
 
I'm wondering about this too. I too read that even if we passed out dodmerb eye exam, we still have to get another. Does anyone know if it will be paid for by USMA?
 
At least I don't have to go through DoDMETS and DoDMERB again.

Is it basically the same vision exam we got earlier? Depth, color, and the whole shabang?

Yes , you have to get one done by your own eye doctor (on your insurance).

The use it to determine if you need glasses and if you, do the Rx (for your very attractive BCG's) will be made from that exam.
 
I have a question so I'll just add it to this thread since it's regarding the Vision Form.

I've taken my vision form to my optometrist twice now to get him to fill out the required sections because the first time he didn't get it all. In the very first part of part 2 (number 4.) it has a chart that says "Spectacle Prescription". But only about half of it he filled out. I don't know anything about eyes or what any of this means, so I don't know how to tell if this is complete based upon my Rx. Must ALL of the sections in this chart be filled out or can some of it be left blank? I'm very confused and have already had to take this in to him twice...

Is there a checkpoint on our candidate profiles for the vision survey? I was looking to see if they have received mine yet (only sent it a few days ago, so it's VERY unlikely...but I will keep checking nonetheless) and where there is a list of all the other paperwork they need (Birth certificate, police record check, etc), I don't see one for the vision survey. Anyone know if it's on there at all and if not how we know if/when it's been received?
 
You only need to fill out part two if you need your own special pair of Birth Control Glasses :biggrin:. Yay 20/10 vision

Yes, I didn't have to schedule an appointment. If I did anything wrong, Admissions hasn't contacted me about it yet, and I sent it in a while ago.
 
Alright, cool. I mean that's the way that I had read it, but we wanted to double check anyways
