mistake on form

Hey, I overlooked one of the questions and marked no when I should have marked yes. Back in 2003 or 2004 (I don't remember exactly), I fractured my ankle but when I read the question "bone or joint problems" I didn't think it counted. I never had surgery and played soccer for all four years in high school and volleyball for one year in high school. It never was a problem.

When my husband deployed, I sprained my ankle and had an x ray done (its in my military records b/c we have tricare) and in the x ray is says there is an incidental os trigonum. I have no clue what it means but I'm thinking its referring to the healed fracture mark? Anyways, is there a way to fix this? I don't want something dumb to happen to my foot down the road and get in trouble. I don't want to lie and I didn't mean to lie, I just made a mistake and overlooked, I guess misread, the question.

What should I do?
 
I haven't looked recently, but did you mark the fracture down for any of the other questions? I think there is one regarding fractures.

If you annotated it for one of the other questions on the form then it's not a problem at all. If you didn't mark it anywhere on the form and you really want to be thorough you can contact the people at DoDMerb and explain that you need to add additional info to your form.

Not a huge deal either way.
 
I read over the form a hundred times and doesn't specify fractures, just any bone or joint problems, injuries or surgeries. I wrote a letter that I am going to attach to my forms as if it were a questionaire (like the allergy one) explaining the situation and I made clear that it has not been any issue since. It was back in 2004. I played four years of soccer 2004-2008 in high school w/o issues.

You left out the ending of question 50:

50. Any bone or joint problem, injuries, surgery or medical treatment

and question 78 may also apply:

78. Had any injury or illness other than those already noted

I think your actions to amend the form will satisfy the requirements, good luck!

My DS accidentally checked yes for something he did not have :) When he noticed he was told to just cross it out and initial it and explain in the comments section under that item it was a mistake. he did and it was not an issue. I would think you would do the same thing if you accidentally clicked no instead of yes as well, and you can clear it up at the actual physical. PS, my son got his Qualified status letter back in about one week.
