How are cadets placed into all of the different companies at West Point? Is it determined plebe year? And are you in the same company for all four years?

All plebes put on the Harry Potter Sorting Hat during Beast. The hat assigns you to a company.
Sorry, I couldn't resist.

My Yearling has been in his company for 2 years. Quite a few buddies from his Beast company ended up in his academic year company. Historically, the Yearlings have scrambled to a new company for their Cow year but I think there have been occassions where they stayed in the same company all four years.

I'm sure there are exceptions, but DS indicated it appears that beast squads/platoons moved together into a company, but not necessarily the same company as they were in during beast. (The numbers would not support that).

Likewise, there seems to have been a conscious effort not to have beast squaddies in the same squad for the academic year first semester.

They scrambled after winter break, changing squads, rooms, sometimes barracks, PL, 1SG, etc. And even "team leader, the yuk in your squad assigned to you as mentor.

as Dixieland indicated, it appears that they only scramble you out of your company as rising cows, and from there all bets are off.

Your beast relationships stick, DS interacts with his old squaddies on a regular basis. During duties for sure, but even for just hanging out.

Makes sense, since it "simulates" what they'll encounter once they are commissioned and in the field.

DS kept one roommate during scramble, but not the other. Moved from "Hot" floor/location (near 1SG) to a "chill" floor much further from inspections and scrutiny.

Which is not always a good thing, he felt he was losing a bit of edge that he had maintained due to proximity to 1SG, so has consciously made an effort to stay squared away even if less likely to get scrutinized. (Inspections, knowledge, duties, etc)

So that is our observation as well, looks like conscious effort to get cadets used to adopting to new circumstances & teammates without destroying some of the camaraderie developed during beast.

Sounds like "mission accomplished" to me!

Some current Cows could stay in the same Regiment if they volunteered to go into the new I companies.

No one knows yet if you will change companies and/or when. My class (2011) changed companies as a plebe. The class of 2009 did not change at all and other years have been different. It keeps changing, so don't expect to know anything for sure. There are pros and cons both ways. I loved scrambling into my new company after Plebe year. I met a lot of great new people.

Initially, you are placed into your Plebe year company based on which Beast squad you are in (each squad goes to the same company together after Beast; so even as a Yuk now who has yet to scramble, I am still surrounded by my Beast squad mates). Others from your Beast company may end up in your company (that wasn't the case for me; all of the 2014 cadets in my company are from different Beast companies).

As mentioned before, the scramble policy is in a dynamic state right now so it's hard to tell how scrambling will work. Based on what we've been told by TACs, West Point tries to scramble people into various companies to try and best achieve a certain level of balance between race, physical performance, academic performance, etc. into the new school year.
