Question regarding remedial

Hello,

I received a letter stating that I need to send over more information
regarding my chest X-Ray.

R251.21 - Please provide additional information regarding chest X-ray, was it due to having positive TB test in past?

I know for a fact that I am negative for TB. However, I do not have any documentation to support it. My school simply asked me to go to the address they gave me to get a chest X-Ray. I did not receive anything, other than a follow up letter telling me that I'm negative for TB. My parents threw away the letter because we didn't think it would be important. I do not have any documentation to send over to DoDMERB to confirm that I'm negative for TB. I have no medical history of having TB. Will a written letter be sufficient enough?

Call or go to the address where you received the XRay and ask to see if they still have copies of the information.

Thanks for the reply. What if they don't have the medical information? I sort of just walked in, got an X-Ray, and walked out.
 
It varies by state, but medical facilities are required to keep patient records for a certain length of time after the last patient visit. In many cases it is 10 years, but many things affect that. Unless you had this chest xray when you were very young, I would say your medical facility probably still has the record and could provide you with a copy. If worse came to worse, could you go have another TB test and prove that you don't have it?
 
I went to the clinic where I got the TB test and X-Ray three years ago and got an official copy of the results (negative for TB). I went ahead and faxed it to DoDMERB, hopefully this gets cleared up!
