Form Deadlines

I am almost certain that there are only two forms due prior to July 1st. The Vision form is due by May 22nd and the Immunization form is due by April 15th. Unless I am mistaken, the rest are due July 1st. Don't procrastinate too much though!
I literally just got that done today. My suggestion would be to call WP's clinic (the number is on the form). According to them, the blood type is the least important part (not like it is going to change anytime soon!) and you can submit the rest of the form minus the blood type, as long as you put an indication stating that it will be updated, and submit a copy afterwards with the blood type and related results attached when you get it.

If I was just offered an appointment yesterday, will the April 15th deadline be different? It think it would be impossible if I haven't received the things yet in the mail!

I received an appointment back in February, but havent accepted it because im still waiting on Air Force and Navy. Is this deadline for people who HAVE accepted their appointment?
Paragraph ! on the immunization form says "... this form must be fled out and returned to West Point as soon as possible." Where did you see a due date of 4/15? Odd that 4/15 would be a deadline since it is a Sunday. My DS will not be making a decision until closer to the end of the month.
Within the Instructions for Candidates offered admissions, there is a breakdown towards the end that details each of the forms, how to fill them out, and their respective due dates. The Immunization form is covered under bullet 6 and states, "this form must be filled and returned to West Point prior to April 15. (Late appointees should return the form as soon as possible.)" Also, after getting the initial copy, two extra photocopies should be made, one for personal records and one to bring with you on R-day.
