Police Record Check - what envelope

I am about to give my police record check form to my local police. Although the overall instructions say to mail all forms using the gray envelopes, the police check form has a seperate address listed on it. What should I give to the police with the form -- one of the gray envelopes, my own envelope, or should I expect my police department to use one of their own? I am not sure since the address listed on the form is different than that on the gray envelope. Thanks in advance.
 
Our DS provided the local police with a regular envelope (not one of the gray envelopes). It was stamped and addressed to the address listed on the form. It worked...the police report is now checked off on his portal.

I used the gray envelope and I have a green check in the portal. So take your pick.
 
Is the "Police Record Check" just that 1-page form about "Background Check"?
 
My cadet left an addressed, stamped envelope with his form. He was told to let the law enforcement agency mail it---it was not to be returned to him. He had to do the city police and sheriff's department.

Read the directions carefully.

And if you have a part time job not in your own town, make sure you know to give the form (and envelope) to the police station in the town where you work as well as live. You may likely have to make 2 or 3 deliveries, and they may have different policies, requirements.
 
today my dd went to all the police stations and the sherrif station. they all handed the completed forms back to her while she waited and the sherrif didn't fill out her sheet they typed a letter stating that she has a clear background.

i hope she did it right. but it doesn't look like it. i guess she can send what she has in and see where it gets her.

She needs to let them mail the information.
 
When we went to our local police station they told us that they don't fill those out. They directed us to the county sheriff's office who filled it out then took the envelope we provided for them sealed it up and stamped the back of the envelope and gave it back to us to mail.
 
fair enough. i'll have her go back by with the forms and envelopes and let them mail it.

Good plan. It may not hurt for your DD (or you as her proxy since she is likely in school during daylight hours) to confirm with USMA Admissions that a letter to file is an adequate replacement for the form itself. USMA seems to like forms ALOT. Lots of forms. :smile: Your DD needs to make sure this gets done properly and expeditiously since I believe this item and fingerprints both need to be closed out because they are prerequisites to starting her e-QIP (Security Clearance) process going, which needs to get completed prior to R-Day.

hi thanks for your response. the acceptance just got here on monday. so we're trying to get up to speed. it actually was lost in the mail!

she took the form back to the sherrif and they couldn't fill it out they had to use their letterhead. she then asked the clerk to write a note to that effect for her and she had them attach the form.
