I'm in the process of packing my backpack and wanted to make sure that I haven't forgotten any important papers. Below is the list of documents I know to bring. It would be much appreciated if anyone notices something missing, if they would please comment.

-SF86 with birth certificate
-Cadet Allotment Form
-Airman-ship documents including: 1042, CPHA Screening, TSWF Encounter, Pharmacy Cadet in-Processing, and Shot Record
-SGLV 8286
-O-143 Emergency Data Form

Also, if any upperclassmen have any tips on what to bring... (or not bring), that would be awesome too! :thumb:

paperwork

Where is the cadet allotment form?? Wasn't that mailed into the bank?

It was something with the AOG if you wanted them to take a monthly rate out of your cadet pay in order to cover the membership fee

Personally, I'm bringing a copy of all paperwork whether it was sent in or not which includes the shots record and other forms.

Better yet, scan and have digital file (i.e google docs, pdf etc.) that you can retrieve and print.

A print copy is much better.

If, for example, the Academy misplaces your shot record and your copy is somewhere in the cloud rather than on your person, you will find yourself with a sore arm. I-day is an assembly line, so you will not have the luxury of being taken to the nearest computer with a printer to get what you need.

Good point. Thanks:thumb:

?

They had us mail in the SGLV 8286, and the o-143 emergency data form didn't they?

I haven't received confirmation on much of anything I sent. Lol. If they don't have it, they will just make you fill out another Thursday right?

As was stated earlier, if you bring one piece of paperwork that was previously sent, make sure you have a copy of your shot record on your person at all times that first day. I will recommend that it is in your wallet/pocket and easily accessible (not in your file where you may have to search for it). You may still get one or two shots but it beats the heck out of 5-10. IMHO :thumb:

Carry copies of all paperwork with you -- put in a zip lock plastic bag -- if you carry in a file folder, something may drop out or you may get nervous and drop your papers all over the floor. Zip lock gallon bag works well! Do not count on being allowed/able to print anything -- have the paperwork with you and accessible.

I have an accordion folder for all my paperwork that is clearly labeled with a post-it note visible when you open the folder. I find that this organizes the paperwork nicely without just being a stack.

If I'm not mistaken, you carry everything on your left hand. A ziplock bag that goes into another plastic USAFA plastic bag is what I've seen most basics carry from previous years. Less is more.

One question, which forms are to be signed and which ones are not?

One question, which forms are to be signed and which ones are not?

Last year the ones that had to be signed at the Academy were things that needed a witness--life insurance (SGLI), security I think. Look closely at the forms--if there is a line for a witness signature next to or under yours, don't sign it ahead of time. By signing it in front of a witness, you are verifying that everything on that form is true or as it should be (level of life insurance coverage, beneficiaries, that all the security questions you answered are true, so you can't come back later and say that it wasn't you that filled it out.) If you don't sign a form, you can always do it there as you turn it in.
