My cadet needs to fill this out. The last column, which follows the "Family History" column, asks for "Current Medications". We're not sure if they're referring to him or to the immediate family! Just want to make sure we get this right, with time running out. Thanks!

I believe it's the current medications of the Cadet. Following that, it asks if the cadet takes multi-vitamins, ect; So, I put my current medications.

Thanks, I looked at that column many times; just didn't seem to be the place to put "cadet" medication info. I guess he'll find out before me! LOL:rolleyes:
 
Another question about the form; if I had a cigar with my dad last week do I have to check "yes" in the section that reads "Do you now or have ever used tobacco products?"

It's best to be honest when answering the forms. If you used tobacco, then you will want to check yes. The Honor Code is "We will not lie, steal or cheat, nor tolerate among us anyone who does. Furthermore, I resolve to do my duty and to live honorably, so help me God." You don't want them thinking that you already broke the honor code before we even arrive, you know?

It's not a question about honesty, I understand the honor code, I'm just wondering if they are referring to habitual use or a one time thing.

I think this is more of a medical issue than an honor code one. Really, the question should be a little more specific about whether they mean continuous past tobacco use. The information could be critical in diagnosing possible illnesses/conditions in the future.
