I completed the medical history survey and when I was reviewing my answers realized I marked a question YES incorrectly. The system will not allow you to make corrections. I can add in the remarks section a statement that I incorrectly marked YES and use a black pen to make a correction on the printed copy. However, I'm concerned this will cause an issue down the road, I don't want to electronically sign this form as it is any suggestions?
 
You might want to call DODMERB and ask them how to handle the situation.
 
Spoke to DoDMERB via phone today and was informed they can't re-set the survey. I was told to not sign it electronically (which will show as being incomplete), print and make changes by hand and discuss with doctor. I'm sure there is a reason for this but it sounds like the system needs updating.
 
Something similar happened to me last year. I hadn't realized I had marked something wrong, and the doctor discovered this error when he was giving me my physical. He just took a pen and changed the marking directly on the printed sheet.
 
When my son completed this on line and finalized he went to print it out to bring with him to the appointment on thursday. It all printed out except for the X in the boxes. He went in and hand completed the Xs. We tried to print it out on a bunch of different computers and printers all with the same results. All of the data and explanations were there-just not the X marks. Hopefully that will be ok.:confused:

We had this problem too. The PDF opens in a browser window. I found that if I right-clicked on the window, then selected "Open in a new window", or "Save..." the resulting new window or saved PDF had all of the checkboxes filled in appropriately.

YMMV, but that worked for us.
