Will the answers on the questionnaire be able to be explained and discussed, or is a yes to a question an automatic DQ? There a couple that I would answer yes to in order to be completely honest and open, however when explained, I do not believe they are a problem at all.
 
Hmm - depends on the question? When my son applied, he reported a mild concussion, but when he filled out the concussion questionnaire that was sent to him, his answers made it clear that it wasn't an issue and so he wasn't dq'd. Another applicant had an MCL strain, reported, and a doctor's letter was sent confirming that the knee had completely healed and no residual issues existed, so no DQ. However, I think if you report diabetes or some other condition that is truly a DQ, then I that would be an automatic DQ.

And there is room on the questionnaire to more fully discuss any answers indicating an issue.
 
I'm am almost completely clear of the medical issues, but the last question where it asks if you have been treated at all for anything other than minor illnesses and the substance uses in the beginning.

During early childhood(before 5 probably), I was hospitalized for some sort of minor respiratory problem where I simply could not breathe very well and needed assistance through a nebulizer. It was only for a couple of days and only occurred once or twice. I have not had issues since then(10+ years have passed).

Also, for the "Do you or have you ever consumed..." questions, what does this question aim to answer? Any alcohol consumption at anytime in your life? or an applicant who parties constantly and gets drinks every weekend? Should you list yes just because you have EVER consumed an alcoholic product before?

Again, when explained to a doctor, I'm sure these issues are not a problem, I am just unsure of what to list on the questionnaire with all these talks of integrity and honesty up front.

If you consumed what they are asking about (that's the problem with incomplete sentences) then you answer yes and explain it where you can. Same with the nebulizer issue. If you answer incorrectly and they find out later (and generally they can, certainly for the nebulize which will show up in your medical records) then its worse than having told them to begin with. Such action makes you unreliable and dishonorable in the eyes.

If you only drank a couple or a few times and are not a "partier" then I expect you'll be OK. They were there once themselves. Keep your nose clean in the future though or you'll be out.
 
I have made a mistake in choosing Yes instead of No answer. The system would not let me to make change, but I did put mentioned about my mistaken wrong answer in Block 83 Remark. Will it cause the issue with DoDMERB?
