Immunization forms

I somehow managed to overlook the May 15 deadline for immunizations and the PPD test. There is no way I will be able to get that test done by then. Anyone know the repercussions of that? How serious is that deadline?

Also, I hope no one else is in the same boat. Getting the immunizations sorted out is a pain.

I would call Nominations and Appointments (Admissions Dept) first thing tomorrow morning and ask them. They'll be able to give you the good gouge. Good luck.
 
Get one placed tomorrow, get it read Wednesday and overnight the form.

Only the original form needs to get sent in by the 15th, according to the instructions which tell you to make 2 copies and bring them with you on I-day, you should bring in any updates to the record, which infers that you can get shots between the 15th and I-day and just bring the additional documentation.

According to our doctor's office, PPD is under a national shortage, and when they called the USNA immunization clinic, USNA told them just to document and USNA would give the test.

Probably the worst and only repercussion is that you might have to get a bunch of shots on I-day that might make you sore, not feel good or you don't need.

The way we read it is the form is due by then...and they require that Tuberculin skin test done before I day....and if your forms shows missing immunizations then you are going to be given tons of them on I Day if you dont have an updated form for them on June 27th. But my DS got his skin test done last Tuesday, read on Thursday and form mailed off Friday. So go get that test and like someone else said...overnight it.

OP what did USNA advise? My DS is in same boat, we are in a rural area and the skin tests were on alternating weeks but only Monday shot with Wed. check. Which made him have to send a overnight on the 15th. We made sure that package had to bee signed on delivery. Didn't know what else to do.
 
My package won't be there until tomorrow. It can't be that huge of a deal - it's a bummer missing the deadline but it will be in the records long before I day regardless. I'm sure we aren't the only ones having trouble getting it in on time.
 
Were we required to send in a separate form for thr ppd test? I did not, but had the information filled out on the immunization forms.
 
i wouldn't sweat this too much. I've heard there may be a national shortage of PPD vials and this is NOT readily available at most pediatricians offices.

From working in an Urgent Care center, the only reason we stocked the vials was for pre-employment of our occupational health services, otherwise, it would have NOT made good financial sense to buy it, as most "normal" folks do not require a PPD test.

If I lived outside a major metro center or far from a public health department, I would just send on the packet with a written explanation and "plan." Your plan would be - already contacted the county or state health department, nearest hospital or even law enforcement - they are often screened as police often deal with homeless and non-nationalized citizens who are at greater risk.

You might have luck with EMS or fire departments - usually deal with the same crowd (higher risk).
