If your DS or DD has had all his immunizations and a recent visit to the dentist, it should not cost anything. Your pediatrician should be able to fax over the immunization records and fill out the form. You will be required to have a polio shot after age 18 which may cost some, but if you do not have this done or if your DS/DD will not be 18 by R-Day, they will give it to you. Additionally, if your DS/DD has not had the Chickenpox vaccine, a titer will be required. Do not know if your pediatrician can verify blood type or if a lab is required. We had the labs done with the varicella titer.
Now that I think about it, included with the grey envelopes and the additional dental form is info about the x-rays. They need bitewings and panarex and it lists how old they can be (can't remember if it's 12 or 18 mos). We are having one set redone and can use the other. You should be good with 7 mos.
