DoDMERB Optical exam question

Just wondering if we need to bring any of the forms that are to be filled out on the dodmets website to the optical exam? Should we bring (for example since I have glasses) info about the diagnosis, etc?

Would you only have to fill out something if their is a problem?

I didn't bring mine and my eye doc didn't ask for them. I brought my glasses, contacts, and my prescriptions for both. The doc said bringing the prescription made it easier to see where my eyes were and where they are now (my eyesight had actually gotten a whole lot better). They do all their own tests so I don't think any further info is necessary. ~They fill out all the paperwork.
 
Bring your glasses and wear them for the depth perception and color vision testing!!!!!!!!!!

Anyone who stumbles here looking for an answer to the above question: You don't have to bring any forms/paperwork etc...they fill it out themselves!
