Appointee Kits -- June 15th Deadline

It is extremely important that all appointees to the class of 2018 fully complete their appointee kits by the June 15th deadline. It is true that not all forms are available yet (but most are, and all the ones that will take any significant time to complete are all available now).

Do not delay completing the immunization form. Virtually 100% of appointees will need additional immunizations and you can't complete the form until you have them. While I'm sure most already have received the Polio vaccine and had a TB test years ago, the DOD requires that you have both of these after January 1st of the year you enter the Academy. That means that most of you will need these two (at a minimum). You also must have at least the first shot in the HepA and HepB series, as well as several other common vaccines. Flu and HPV are optional (but recommended).

Another form that takes quite some time to complete is the security questionnaire. It is a 127-page fillable PDF. Don't wait until the last minute (get it done ASAP).

There are others that require some time and thought, so we *strongly* advise that you work through every available form and do them promptly. USAFA is serious about revoking travel orders and rescinding appointments for those that don't complete their appointee kits.

DD got her TB tine test last week. She had trouble finding a doctor's office that carried it. She was told that there is a NATIONAL SHORTAGE. Plus this test has to be "read" between 48-72 hrs of receiving the TB shot on your forearm. Please plan accordingly. Good luck and God bless you all.

Everything now available has been completed on my portal. Many forms have been completed for a while now and still aren't processed. The earliest date I see is 4/18/2014.

Will it matter if they are processed by that date? And any info on when things are expected be shown as received and processed after submitted?

All USAFA needs is for you to submit the forms. The processing is our issue.

This is a brand new process and we are still working through all the details. We appreciate your efforts in getting the forms completed!

In DS's view it would have helped having the Immunization and Security Forms opened up first to work as they take the longest. Bringing in the I-Form now puts a little snag in getting a Doc-Appt quick enough to do it all in good form. But, its a good learning experience.

Thank you for the feedback. We anticipate all forms will be ready when appointee kits go live next year so hopefully it won't be an issue for future classes.

We had noticed a definite "stall" on form completion here. The text we sent was meant to get attention (and it did work). Everyone still has ample time to get everything done but we did feel it necessary to raise the attention level or a number of appointees may find themselves running short of time.

In short, there is no need to panic but do get going on the long-lead items (immunizations, security form, and Uniform Issue).

The Appointee Kit was updated today to further clarify some areas of confusion, to highlight "long lead" actions, and to indicate which forms are due ahead of the overall 15 June deadline.

These forms/actions have a deadline of 1 June:

1) T-Shirt Size
2) Medical Statement
3) Travel Options
4) Optical Declaration

These forms are due earlier to allow USAFA time to process orders for shirts and glasses, evaluate medical responses that may need referral to DODMERB, and book airline tickets.

If you haven't yet been assigned a PO Box it's because you haven't completed your travel election. PO Boxes are generated at the same time as travel orders (which can't be done until your travel option is processed).

The "Airmanship Programs/Medical Screening" form should be available later today or early tomorrow.

Even if a Requirement has been previously completed,albeit maybe not Processed, the topics on the Portal are in Red regardless?

Yes. It's for information only, not to indicate that you are late or otherwise haven't completed the form.

I chose the driving option when filling out the travel options initially, but my plans have changed to air travel. Do I need to inform someone, or do I just wait until I - Day while filling out the travel paperwork?

DD received a waiver for an ACL tear. She is required to receive "check on entry." The directions on the Medical Screening Packet says that she should pay attention to "any special requirements set forth with such notification."

This is what it says: Please bring all medical records
for the above condition that you haven't already provided with you for this check.

We must have already sent a ream of paper to DoDMERB. What other medical records would be needed? I have an electronic copy of the xrays and MRIs, I suppose she could bring those?

You are correct. You only need to bring the pages you filled in.

Since you are buying the ticket yourself, there is nothing you need to do now. Bring the receipt for your airline ticket with you (you'll need it when you file your travel voucher).

Err on the side of caution here and bring everything you have with you. It's the 10th Med Squadron that will make the call (not DODMERB).

How do you suggest that I print out a copy of my immunizations?

and will I be required to get my TB and Hep shots done despite having had the shots last august?

I've called admissions multiple times with prior enlisted specific questions and they haven't told me any more than read your packet, which I have already read through... I wish that they would take the time to truly answer my questions

Call the Immunization Clinic at USAFA. They should be able to retrieve your records and tell you the dates you received the shots.

Immunization Question

The information in the instruction for appointees from the booklet says that a questionnaire with required immunizations will be available on the portal. Appointees will need to complete part I of the questionnaire, and a licensed primary care physician, nurse, etc completes part II. Is this old information because it looks like we just put in the dates of the immunizations on the portal. I didn't see a questionnaire that you can print and send NO LATER THAN 1 June.

What immunization verification is required now?
