MS1 Paperwork time sensitive...

Hi All, Son will be starting college and AROTC in the fall. He hasn't heard at all from battalion (new PMS on the way I think....) anyways his school college physical is tomorrow. I would imagine the army has some dr's forms that need to be filled out for contracting and we would like to bring the form to his appointment tomorrow (the dr is terrible about filling out forms after the fact!).

Has anyone gotten a copy of the form sent to you? or a link to it? I have spent forever on an internet search for it but no luck, and son is not motivated to hunt down new cadre...

The Lieutenant who is the Freshman Adviser is usually in charge of this stuff, at least at many units. Use the PMed link above or contact that guy... the forms for the physical shouldn't change regardless of anything else swirling around it in the unit.

Does your son have a AROTC Scholarship 4 or 3 year? If he has a 4 year then the Dodmerb physical should be all he needs.

The Battalion HR person is usually the one that handles the bulk of the paperwork, your son should try and get in contact with that person. Every battalion does their paperwork differently, some want most of it completed before they arrive if they are on scholarship so they can contract as soon as possible.

If your son is not starting out on a scholarship there will be little paperwork he will need to have completed before he arrives other then the physical.
 
DS completed the SF-86 form prior to arriving. It is a long form and has many questions your cadet may not know the answer to. I would advise completing that form at home and bringing with them. Just my .0001 cents worth. Also I don't think its that unusal to not hear from the unit. DS had almost no contact prior to his arrival and his first interaction was the APFT. :thumb:

There was one piece of paper, however, the 'Medical Fitness Statement (DA 3425-R)' that looks like it has to be signed by a Dr. Even if he doesnt need it, it probably makes sense to CYA instead of going back to the dr for a second visit. Though honestly, I am sincerely clueless. (for real, this isnt sarcasm, im totally clueless on this stuff)

navy son had to fill that out last yr (or something similar) PITA! I am hoping arotc son gets a contact by the new cadre but he still has a couple of weeks before school starts for that so no worries. He will get that done. Thanks for the reminder!

Has your son started work on the 104R form. Even if he hasn't received one from the battalion, have him google a current copy and start making a rough draft. Hell need to work with his college advisor to get some help in filling it out. Tell him not to worry about it being a set in stone document, they understand it can change as he progresses through school, they just want to make sure he has a plan that shows he can graduate in the required time frame. There are other forms, tax forms, direct deposit forms and such, if he can get in touch with the HR he may be able to get some of the forms sent to him so he can get a head start.

Getting as much done early can help in getting the contract signed, the 104R is one of the forms that needs to be completed prior to contracting. The sooner he contracts the sooner he will start getting his stipend. They will pay the tuition and book money retroactive, the stipend starts the day he signs the contract, they don't pay any stipend retroactive.

Some battalions will just have time set aside during orientation to fill out all the paperwork, getting familiar early with the 104R would still be a big help because the school advisors can be pretty busy the start of school.

I'll refer to Sheriff3 on the SF-86, neither of my sons were required to have that completed before they got to school, that's something that's new in the past couple years. They just filled theirs out at school.
 
^It may also be a good idea if your son has a working knowledge of how the 104R works.

Maybe it's not too big an issue for smaller schools, but at least for me (PSU) my advisor didn't quite understand/know what a 104R is (I think she was also getting it mixed up with the other branches' equivalent paperwork).

In other words, try to request an advisor who knows about ROTC and how to handle the associated paperwork.

... if not, then all hope is lost and the world will end shortly. JK. All jokes aside, it's not too big a problem if the advisor doesn't know - it just may take a little longer to get things squared away.

If you/your son has questions about the 104R feel free to ask me; I've been through it many a times this past year :thumb:

LOL yup kinnem I've bucked this bronco before. My NAVY mid is currently finishing his freshman (plebe) year "slumming" with ARMY cadets at cadet field training (CFT) for his "MARINE Corps Special Opportunities Training" (MARSOT)....figure that one out...the weekly messages from him say they ran out of toilet paper weeks ago,it is 100 degrees and raining every day and they've learned the army phrase..."If it aint raining, then we aint training!" ...., they are out of real food and only get MREs, and there for about a month with no way to clean your clothes...just a little vaca in the Adirondacks....to be honest the texts are littered with swear words I can't even decipher...

In our Battalion the primary requirement is to make sure the student brings an original birth certificate and their Social Security card. The originals will be certified, copied, and returned.

The 104-r is a good idea but it is difficult to understand so you might wanna wait. Once again I defer to my experience at our Battalion, we have a two day orientation we complete with incoming students. The orientation's focus is to prepare the incoming students by assisting with administration requirements and assigning them a Cadet mentor. As stated in my intro I will help answer questions when I am able.
