Can't get physical form for scholarship application - deadline soon!

Our son is applying for an ROTC scholarship. He's completed the application and just needs to do the PMS interview and submit the physical test form.
The deadline for completing those final steps is Feb. 28.
He's been in touch with our nearest PMS and we don't think it'll be a problem getting the interview in before the end of the month.
The problem has been the physical test form. His high school physical education teacher already administered the rquired tests and noted his results. But she's been unable to receive the official form via email due to some firewall issues at the school.
Our son's reached out several times via the scholarship application website help function; they just keep trying to send the form by email, even though he's told them it's not working.
They won't send the form directly to him or us. What other options do we have?
He's getting nervous because it's now less than 3 weeks until the deadline.
 
Is there an option to download the form from the application site. In the past the form was included as part of the application, the applicant could download the form and have the person giving the PFT fill it out and send it in. Do they now only email the form directly to the person giving the test?

Can the Phys Ed teacher give them a personal email to have the form sent, to avoid the school's firewall?

We'll take another, closer look at the website, but I don't believe we could find an option to download the form when we were looking for it previously.

As for the teacher's email address, the school has a policy that prevents teachers from giving students or their families their personal email addresses.

Make sure you give a couple days in between your upload date and the deadline for approval. Best of luck to your son!

Edit: I downloaded the file and printed it out, then uploaded it to the file upload section with no trouble. CC's rather flexible in regards to such

Glad you were able to get the information you needed.

This is a great forum, very active, to ask questions and get information, much better then the other forum you used. Search through some of the more recent posts and ask questions as your son moves through the process. Once he starts ROTC he and you will find that there are a lot of great tips and information that can help navigate ROTC. This is a great place to find out about current and future changes in ROTC.

Best of luck to your son, hope to see you both around here for a while.
