It varies by school. I talked to some people at Boys State and some said they were simply asked by a teacher if they wanted to go and said yes, while others said that they had to go through a selective process and not everyone that wanted to go got to go.Can you ask them to nominate you? Or how's that work?

Hello there. I had the wonderful experience of attending boy's state this year in California. My school had no idea what boys state was, which I thought was weird. But thankfully, i had been volunteering at my local legion during fundraisers and just asked to be sent. They happily obliged, granted no one else from my school applied. It actually depends on the Boys State, but you are right in that you do have to attend Boys State to attend Boys Nation. Some states do it the way you said where the 2 highest positions get to go, but some states conduct interviews to decide who goes and the 2 highest positions don't necessarily get to go to Boys Nation.Boys Nation maybe? If I'm not mistaken in order to do Boy's Nation you have to be elected to the 2 highest position in Boy's State. You will then represent your state at Boy's Nation. Can't do Nation without going to State first.
