AFROTC Activity Sheet... Help!

I've just scheduled my HSSP Interview at my local AFROTC (for Jan 8, 2016). I was told, through email, that I needed to email them back my "Extracurricular Activity Sheet" ASAP. At first, I was under the impression that I could just log into my online application, go to my activities page, and print it off. But there is no option to do that. I assume I will have to make one from scratch, but I've hit a snag...
I don't know what sort of format to use. I know it needs to be a decent and easy to read sheet, if not impressive; but I'm not sure how to organize it or if I should just make it a list or I don't even know what. I've been told to bring my resume separately, and that already has a good summary of my extracurriculars. So I'm worried about bringing 2 of the same exact things, really...
This probably seems like a pretty trivial problem. But I'm really worried about it. I don't want to walk in there seeming unprepared.
So any help or tips at all would be greatly appreciated! Please!
In addition, if I'm going to make a sheet entirely focused on my extracurriculars, is there anything extra I should or should not put on it?
I pretty much have no sports to back me up (which I'm praying does not automatically doom me). I've done almost entirely volunteer work throughout high school (including leadership roles in my youth group), and I have taken care of family responsibilities (bringing up my special needs nephew for 8 years, and then newborn niece for 3, etc.); but my family lives on a ranch and I've had to help put up fences, build sheds, take care of animals, and move 200 lbs hay bales... Does that count for anything?
The "physical/athlete" side of my application is not very strong, officially... I really, really need some guidance. Please, any help would be miraculous at this point. Thanks so much, y'all.

I brought my resume which had the extracurriculars as you stated. It was fine. If anything use a resume format or something simple in that manner.

I know a bunch of people who won without sports but it isn't sports, it's leadership that helps your application. It's one thing to participate in a sport but to lead a team in one is different.

P.S if your PFA score is high, I think it demonstrates your althletic side.
 
Yeah I brought two sheets for my interview. One was my activity sheet (it was in the exact format of the one online) and my actual resume.

On my resume I had no sports or clubs listed (they were on my activity sheet) I only had a list of specific events where I was a leader. I could PM you what my resume looked like if you want and give you some advice. I was able to get a 100% on the interview and was offered a CLS at the end. The MOST important part is to highlight your leadership experience and capabilities. Also remember the three core values of AFROTC: Integrity first, Service before Self, and Excellence in all we do. You won't be expected to know them, it just helps to have them in mind.

Don't worry if it looks like it's going bad at first and your interviewer doesn't look interested. That's what I thought at first, but it all turned out great

That would be awesome if you could PM me what yours looked like. I'm not positive what mine should look like, I feel like I might've mixed both the activity sheet and the resume together at this point. Any example would help a ton. I definitely want to get across that I have leadership skills, but I also want to infer that even though I wasn't doing sports, I was still busy. Thanks, man.
