Braces Question

Hi,
I am aware that having braces will be a DQ, but if you fill out the paperwork with your orthodontist stating they will be removed before I day that you will not be DQ'd. I have filled out the paperwork and had it signed, but I do not know where to fax it to. Could anyone provide the address for me?
 
Double check with your BGO but I believe you can have orthodontics during your DoDMERB as long as they're off by I-Day. Best wishes.

My DS's DODMERB was changed from remedial to pass for USNA after he submitted the orthodontics form indicating that braces would be completed before I day.

The form is available on the DODMERB site.
 
....bump. Both of the previous posts where not relevant at all. I already have the paperwork out. I need to know where to send it. The dodmerb website is not clear.
 
DoDMERB DQs, commissioning sources waive. They have nothing to do with waivers. Have you had your exam and received the DQ or are you just trying to get in front of it before you get the DQ?

Your post reads to me that you have yet to take the exam. My guess is that if that is true, just take it to your exam and have them submit it formally with your exam results.
 
I already had my exam done, I want so submit the paperwork so it will speed up the waiver process or even avoid it all together.
 
The following is from the DODMERB FAQ's. Our DS also used certified mail to send his orthodontia form, and we sent it certified mail to the USAFA address on Edgerton. We chose to mail it just in case DODMERB wanted an original signature from the DDS. I also have a personal aversion towards FAX machines. Regardless, the remedial was eliminated after we sent it to this address.


10. Will you allow me to fax documents pertaining to my medical status to you?

You may fax documents pertaining to your medical status to DoDMERB.

We do ask however, if the total number of pages exceeds 15, that you mail those instead of faxing. DoDMERB has limited ability to receive faxes and faxes exceeding 15 pages could delay our ability to process your documents as quickly as possible.

Please note: If you fax documentation to DoDMERB, you DO NOT need to mail the same document(s). Our fax number is: 719-333-3578. Our mailing address is: DoDMERB, 8034 Edgerton Drive, Suite 132, United States Air Force Academy, CO 80840-2200. to gokings814: on the orthodontic questionnaire and based on your experience, can the date of braces removal and date of the end of all active care be the month prior to I Day? For example, if braces were removed and treatment ends in May,2015, would that cause the remedial to change to a pass once the questionnaire was submitted? Or, must the date be in April or before to cause the remedial to change to a pass once the questionnaire is submitted? Also, are removable and fixed retainers allowed after I Day? Thanks.
 
Sorry, I never saw your post until now.

I believe you will be okay as long as you arrive at I day without orthodontia and that your remedial has been approved.

Check with your BGO to be sure.
