If this is for the eventual nomination and steps in between, first step for your DS is to read every related page, link and drop down on relevant MOC sites. He may find a sample resume there, such as the one below, first hit on a Google search on "service academy resume formats samples templates":

https://benishek.house.gov/sites/be...MilitaryAcademyNominations_MasterDocument.pdf

It's all out there, as many thousands go through this every year.

Take a look at this thread http://www.serviceacademyforums.com...put-on-my-army-rotc-resume.51268/#post-506353
I would suggest 1 page. DS used the following categories.
Academics - Including GPA (weighted and unweighted), class rank, SAT scores, AP test scores etc.
Leadership - (community service was rolled into this section) and included EAs and Scouts. Summer job was rolled into this section also.
Athletics - emphasis was on Leadership through Sports
Awards

The USNA and USAFA 'resume' is an on-line form and is mostly point and click for sports, work, leadership and extracurriculars. I think 5Day has the broad categories defined, but unless USMA is vastly different - you do not need to worry about the 'resume format' that is actually submitted as part of the application as it is prescribed.

I assumed that that the resume was needed for MOC nomination application and/or interviews and was more free form.
