Hey all, I just got back from my optical exam and everything went well (Medical is in mid August). I have one problem though, after my optical exam the optician collected the form that has all the slots for optical and medical examiners to fill out (form 2351) and said she would mail it off. However, I was under the impression that I was supposed to get this form back and take it to my medical examination as well to get them to fill out the rest of the information. Am I supposed to print out another copy for my medical exam? Or when DoDMERB receives this form from my optician, will they forward it to my medical exam as well? :confused:

Another question: When DoDMERB is collecting information for waivers and disqualifying conditions, do they ask your medical provider to forward your entire medical record/history or do they just ask them for information regarding that one issue?

You really weren't supposed to bring that form to your optical exam -- they should have had their own paperwork to fill out. At least, that's what they did in my case. The paperwork only had to be brought to your medical exam. If I were you, I would just print out another form from the DoDMETS/Concorde site when you go to your exam. I wouldn't want to leave it in anyone else's hands, personally.


If you have a disqualifying issue, I believe they will ask for a remedial first. I'm not quite sure with the whole waiver/remedial process, so someone could clarify on this. Anyway, I have a disqualifying issue, and DoDMERB has requested a remedial:

DOES NOT REQUIRE A PHYSICIAN'S APPOINTMENT: Please provide copies (NOT ORIGINALS) of ALL treatment and/or hospitalization records regarding XXXX , to include ALL applicable inpatient and outpatient records including, but not limited to, VA disability/examination records, DD Form 214, operative reports, anesthesia records, pathology reports, follow-up visits, rehabilitation records, recovery room records, x-ray reports, etc.
 
Thanks. When you had a DQ did they email you stating that? And are you supposed to provide all information regarding that or is that your doctor's responsibility?
 
I did not get an email. I checked the DoDMERB website (which you'll use after the Concorde site says that everything has been sent). On the DoDMERB site, it says "15-JUL-09 Date letter sent - remedial." I haven't received a letter yet, so I can't be certain. However, I'm assuming they are not sending that to the doctor provided by Concorde, because they most likely are not your primary care physician and do not have access to your medical records. I'm asking Mr. Mullen, too, so I'll confirm in just a bit.

The Optometrist uses their own form, which they send back to Concorde:thumb: I did NOT work last night (tired) but I plan on responding to all emails that were sent yesterday and today....tonight.:thumb:
 
The packet sent only has two forms. DD Form 2492 and Form 2351. What form is supposed to be provided for the optometry exam? I don't see anywhere in the instructions to print an additional form for the optomotrist.

So we never see the results from the optomotrist? The only paperwork she gave to my son is a depth perception work up. The flight dr/flight med tech filled 3-4 of the fields of the optometry portion of the 2351 wrong. We hope to have a followup on Monday in that office.

In addition, the Dr was on a reserve rotation, and has left, the initial appt was July 21. I thought we needed the optometry appt b4 he could sign off, so we went back July 30 after the opto appt and he was gone. The paperwork is not signed, so not sure what will actually happen on Monday.
 
homewith4 - my answer does NOT pertain to your son. You obviously did not use our Contractor (Concorde) for the exam. You used a military treatment facility (MTF). Pls send me your posting; full name and last 4 SSN of your son; to Larry.Mullen@dodmerb.tma.osd.mil. I can assist you better from there.:thumb:
