Got remedial R251.24?

What does this usually mean?
I saw this online, but haven't gotten the letter yet.

It requires all outpatient records from all doctors/clinics/er visits/ etc that you received since age 10. It is a pain (and expensive) to get all those records but obviosly important because they must be questioning something in your application.
 
Update:While it was a pain, it wasn't expensive. If anyone else has to do this, mention that it is for _____ ROTC or ____ Academy.
I got mine for free because I mentioned the Army ROTC.

Sometimes that works, sometimes it doesn't. Hopefully it will work for most everyone.:thumb:

I know this is an old post. but do you need to include all dental and eye exams and what about mri's and x-rays?

what if dr made mistakes on daughter's medical history

i called the help desk and found out today that you don't need to send in eye exams or dental exams or actual x-rays for the R251.24. so that's clear to me.

i do have another quesition though. i read one of her dr.s notes and he wrote that she had migraines which she doesn't and he said she had a history of asthma since 2000. she doens't have asthma and the same dr wrote a note stating that my kid has no asthma. should i write a note disputing this incorrect info or just wait for dodmerb to contact me? i've called the dr to see if he would correct this and so far he hasn't responded.

any good advice would be appreciated.
