I'm not sure if this has been resolved yet, but you want to go to a psychiatrist or other qualified doc and have them administer a "Multidimensional Anxiety Scale and the Personality Assessment Inventory" and be prepared to submit that as AMI.

old thread, but... D3 still does not really have official visits like D1 and D2 schools do - this is why D3 recruits don't need to register with the NCAA eligibility center.
Also, D1 and D2 official visits require that compliance paperwork is sent to the NCAA for the prospect. D3 does not visit...
