Bottom Retainer and Form?

From what I read a bottom retainer is fine (been in for 3 years after braces). But do I need to fill out the orthodontic questionnaire for ROTC. I didn't see any form of the sort on DoDMETS. Have completed the vision and auditory screening, next is the medical exam. If we need the form filled out saying treatment for teeth (braces) was completed 3 years ago I rather get that done and bring it with us.
Or just go get the physical?
 
The Orthodontic form is on the DODMERB site, not DODMETS. DODMETS wont issue you this form until, you receive a remedial. However, the form is available to the public without even logging in.

My DS just went to the physical and filled out the questionnaire. He later received a remedial on DODMERB to then complete the orthodontic questionnaire. We took that form to the orthodontist and faxed it in. Within a week of receipt of the second questionnaire (basically stating that DS would be out of braces by April 2015 for USNA), his DODMRB status changed from "remedial" to "qualified." It was a pretty straightforward process.

I say download the form and have your orthodontist pre-fill it out. Keep the form on standby and go do your physical exam. When the time comes IF you receive a request for a remedial, you will be locked and loaded.
 
Thanks we did this, it was filled and it is now saved as a .pdf. Probably will take the form along with us just in case, if not we are ready.
 
I was considered "Qualified" by DODMERB back in December, but I do not remember filling out an orthodontic questionnaire. At my physical I was just asked if I had worn braces (4 years ago) and wear a retainer (which I have for 4 years). Should I be worried that I did not have to file a separate questionnaire?
 
chiefsnow- As long as you correctly filled out your DODMERB paperwork you would have mentioned that you had worn braces, given the Orthodontist information and years of treatment with the conclusion of braces being removed and wearing a retainer to sleep, perhaps even a permanent retainer on your bottom teeth. If there was problem with this or additional paperwork required they would have requested that you provide them before considering you "Qualified". I would think you are fine.
