Look at the letter DODMERB sent you for your Additional medical information. It gives you an email address, fax line, and a postal address to send your AMI to. If you haven't received the letter specifying this, call DODMERB and they will direct you

I second AJC's comment about contacting your technician at DODMERB. Mine helped me with explaining the process and giving me any information I needed

Be careful with your self-imposed diet. It can spiral out of control quickly. Food is fuel.
