The orthodontic waiver is only for people who currently have something glued to their teeth (hence, the need for a wavier). If you had braces, but they are off now, you do not need a wavier. If he never had braces, he definitely does not need this form :)

The email says that it is only being sent to those who have accepted their appointments. Also, it seems like it is being sent either individually or in very small batches (everyone on this board has received it at a different time/day), so be patient!

It says you can't print it out, save it or discuss its content with anyone, sorry... Remember that the whole point of these tests is to place you in the class you need to be in by identifying what you do and don't know. If you do the best you can, things will work out the way they should!
