Yes, I didn't have to schedule an appointment. If I did anything wrong, Admissions hasn't contacted me about it yet, and I sent it in a while ago.

I contacted admissions, and they said I only needed to fill out part 1 if I don't need glasses.

Is it possible to validate a course simply by doing really well on the tests given at West Point, without any AP or college credit?

It's due the October/November of your senior year, but it's good to have it done earlier. Your MOCs should have their website with updated instructions for your class of 2016 between April-June.

Yes, I agree - there are no forms in the document. Last year's document (which was previously posted in October) had the forms within the document itself. But this one does not.
