Academy Transcript Form

On the USAFA Application it states that you must submit a high school transcript, school profile, and senior year schedule. It then allows me to print out a two page "Request for Secondary School Transcript" form. My question is this: I can have my counselor send my high school transcript directly to the Academy Admissions Office. Is the page the Academy lets us print out a format for a transcript? Do we need to use that or can we just send our high school transcript directly to the admissions office. Basically I don't understand what the first page of the online form is for. It looks to have almost all of the information my regular transcript has so I don't know if I need to have my counselor fill it out or if I can just send my official school transcript. I understand that the second page has the school profile format as well as a place to put your senior year schedule (I will have my counselor fill out the second page), but I still don't understand if I should have my counselor fill out the first page of the online printout.

The online printout also wants the transcript to "reflect test results such as ACTs and SATs." I was planning on having my scores sent directly to the Academy via the testing websites. Do I need to include my test scores on the transcript or can I just send them separately to the admissions office.

I know I was pretty wordy with my question, but I am a bit confused. Any help would be appreciated.

I recall that your HS also get the scores automatically, and will prob include them in the transcript. But stop by the HS guidance office and ask.
I'm sure the academy will want a transcript direct from the school, not from you.

When our DS went through the process the transcript had to be sealed. We sent it, the GC did not. The HS stamped the back of the envelope with their name, and than placed tape over it, thus it could not be tampered with at all.

In the transcript it included the following:
1. All classes ever taken
2. All PSAT/SAT/ACT/AP scores and dates
3. Class rank and percentile.
4. Amount of APs offered and pre-reqs required to take Honors or APs
5. Grading scale. I.E. 7 pts or 10 pts for A, B, etc
6. Grading weight for AP and Honors. 4.0, 4.5, 5.0, 6.0
7. Every sport, club organization you officially belong to at the school. I.E. no way to lie that you were in the FHS, when they have no record of it.

The GC report is different because they give more info regarding the school profile.
1. How many go to Ivy vs 4 yr vs 2 yr
~~~~ This in tandem with rank tells them how competitive the school is from a college selection perspective.

I.E. You could be ranked number 50 out of 250, or top 20%, but if 35% go IVY than they don't hand out A's like candy on Halloween. However, if you are ranked 50 out of 250, and 0% go Ivy, the school is not as competitive.

2. Your course curriculum choices.

I.E. Your school requires 4 yrs of PE, that means you probably have less APs than the school that requires 1 yr.
Now if you chose to take 4 yrs and not just 1, it will be discovered here.

Your GC will fill this out on line via their own AFA portal, and you will never see what they post.

I know about what the GC had to fill because she told me. I know what the sealed transcript looked like because I ordered an extra one to see what was included.

So the form I printed out online on my Academy portal... It is two pages. The first page has transcript information like you said, grades, rank etc. The second page is the profile you are talking about. I just don't understand if I need to give my GC both the pages if all the transcript material was submitted via my actual school transcript. This is hard to explain.. But do I need to give my counselor both pages I printed out online, and also send them an additional transcript from my school?

So my counselor has her own portal? I did not know that; as of now the online application only has the two page printout that says it's supposed to be given to my counselor. Again, the second page of the form has the information about the school profile. What is this counselor online portal you are talking about?

I was planning on sending my ACT separately from my official transcript in order to only include my best scores. In other words, I don't want all of my past PSAT scores, ACT scores, etc. to be given the the Academy. I was under the impression that I could officially send only the scores I want to send to the admissions office electronically via the ACT website. Do I really need to include every score of every test I have taken on my transcript?
 
Things change yr to yr. For our DS, you had to submit their(GC) email address.

The AFA superscores it is to your advantage to submit all SAT/ACT scores. PSAT doesn't matter at all.

I.E
May:
680 M, 660 V = 1340
June:
670 M, 680 V = 1350
Sept 700 M, 660 V = 1360

Your score for the AFA would be 700 M, 680 V = 1380.

For AFROTC scholarship they would take your Sept score, because it is best sitting. Thus, your score would be 1360, not 1380.

You are missing the point. If you placed your HS when you registered for the SAT/ACT, the scores will be on your official transcripts. You can't have it removed. It is on their record.

More importantly, it doesn't matter because the AFA superscores.
 
It has been a long time since I applied, but some things don't change.

If it says "print this and take it to your GC" or something along those lines, just do that. The application process isn't meant to confuse you. Follow whatever directions there are. It is quite possible the Academy will get the same information from multiple sources. That is their problem/choice, so you don't need to worry about it.
