If I accept via the online web page for my NROTC scholarship I received, then I do NOT have to send in the questionnaire? Is this correct? I know I read that somewhere I just want to confirm. Thanks.

Here is what DS received by e-mail after he was offered NROTC, hopefully you have received the same:

You can easily accept or decline your scholarship via the website, by email,
or by mailing in your scholarship acceptance questionnaire. If you mail
in the questionnaire, you will not receive confirmation that your
questionnaire was received. You can check the status page of the website
for confirmation. The website will state:

"We have received information that you have accepted (or declined) the
    scholarship."

No you don't have to return the questionnaire if you have accepted online. Only send in one way.
