My son received a R251.22 administrative remedial that doesn't require a physician's appointment. But it said to "PLEASE send a doctors note once your tinea pedis (athletes foot) has been resolved.":confused: How are we supposed to get a doctors note without the doctor seeing him?
 
I'm not sure how your relationship with your doctor is, but for stuff like this I usually just call the office and they have a note ready for me to pick up.
 
Basically that means that you need to see YOUR doctor once this is cleared up, get a note saying that it is cleared up and there are no limitations on performance. Make sure the letter states applicants full name and social security number (or last 4 digits).

This is as opposed to having to go to an approved DODMERB doctor. :thumb:
 
Thanks for the advice - he ended up having to see our physician who wrote a letter to clear him making sure his SS# and name were on the letter.
