Which forms am I supposed to fill out for the the waiver/additional info? I was looking on the download list and Im not sure which one. I thought that DoDmerb would email me a specific form, but I guess I have to figure that one out on my own.

I called DodMERB to ask a question about additional info they requested for my DS. I got a live person who was very nice and helpful. She gave me instructions and also a fax nuimber so that I could send the information to them quicker than regular mail. Within a few days we had a response and a follow-up letter in the mail. Try that if you aren't sure. Good luck.
