Appointee Kits -- June 15th Deadline

The information in the instruction for appointees from the booklet says that a questionnaire with required immunizations will be available on the portal. Appointees will need to complete part I of the questionnaire, and a licensed primary care physician, nurse, etc completes part II. Is this old information because it looks like we just put in the dates of the immunizations on the portal. I didn't see a questionnaire that you can print and send NO LATER THAN 1 June.

Yes it is old information. You need to enter the dates yourself, and bring your immunization records with you when you report to USAFA. Your data will be verified during your medical screening.

The Cadet Sponsor Program form is now available to complete in the Appointee Kit. All cadets are required to participate in the sponsor program.

Err on the side of caution here and bring everything you have with you. It's the 10th Med Squadron that will make the call (not DODMERB).

Insider is absolutely right. DD called the 10th Med Squadron and confirmed that you should bring all your hard copy medical records that pertain to your injury. If you have soft copies, bring those as well since they won't have to scan them and you'll save them some time later on (not required, though.)

Again - bring paper copies of your records since the doctors will most likely not have a computer available to look at them when they check you out.

Squadron assignments are made mid-June and will appear on your portal (provided your appointee kit forms are all done). Report times will be posted at the same time.

The USAFA Clinic need for you to complete several forms and bring them with you on in-processing day. These forms will be examined and collected during your medical screening.

Received / Not processed.

In this part of the Portal the AFA is looking only that you Printed it out, and all will bring it completed to In-processing, yes? Its a little confusing inside when opened it appears they want to have it filled on the screen. When he tried to enter data on the Form it didn't take. Its getting big in the window now, he wanted to ask to be accurate. Thank you...

Yes, the Airmanship Programs/Medical Screening forms need to be printed (and then filled in by hand). These forms will be collected during the medical screening portion of in-processing.

Transcripts

In the Instructions to Appointees booklet, it says all transcripts must be received by June 9th for eligibility. My school will not send final transcripts until June 9th.

Is it okay if they are late?
 
I assume you are talking about a final high school transcript? If so, USAFA is mostly concerned with college transcripts (that is, appointees who are currently in college). Appointments may be conditional on receiving a final college transcript but not on receiving a final HS transcript. USAFA knows that some high schools (particularly in the northeast) don't finish until well into June.

So the answer to your questions is no, it is not a problem (so long as it is a HS transcript). If it is for college then you will need to call your counselor.

Suggest you keep a paper AND electronic copy of your security forms.. This won't be the first time you see this form.. I believe you are required to complete it about every five years.. and any discrepancies may raise some red flags.

What is the correct laundry mark? The Instructions to Appointees book just says first initial of student ID. That doesn't seem correct. So is it first initial of last name and last 4 of SSN, or the student ID listed in the portal? Thanks!

It is the StudentID. USAFA is eliminating using the SSAN (or any part of it) as an identifier per SAF and DOD directive.

Yes, I had the same question. It does seem long to write inside a pair of underwear or collar of undershirt. Obviously will do as directed though!
 
Mine reply was a question, not a statement of fact. I'm still trying to get the definitive structure for marking.

Are others 'Airmanship' part of the Appointee Kit still not Processsed by the AFA? Last one for my DS, other than 'To Bring'. He wants to get the 'Ok to Travel' link live before Graduation Friday and then heading off to Senior Week Saturday!
