Appointment revoked if items not submitted?

I have two items that are waiting to be submitted. My tattoo form is currently with my ALO, and my Immunization record was returned because I need to attach my PPD documentation. If I don't have it all submitted, would I lose my appointment? I am asked to have it completed by tomorrow.
 
Remind your ALO of the deadline.

What's the holdup with your PPD docs?
 
I'd ask why the ALO has the form? I have my candidate come see me...he/she signs the form in my presence, I then sign my part, and they take the form with them to submit.

I'd be on the ALO!
