Authorization for DoD to discuss ...

    Can someone give me an idea as to whom I am supposed to authorize ? Is this for me to allow the DoD to share my medical info with my parents or with the Academy and ROTC ? If the later, how do I find out who the pertinent person is ?
    I finished my medical exam yesterday and would like to send all my info out to DoDMERB in Colorado via DHL on monday, but am a bit stumped as to how to fill out this form.

    Did they not send instructions?

    Is it a boiler plate paper...basically sign at the bottom and date. May have a 2nd signature saying if under 18 parent/guardian signature.

    Usually this form exists because of multiple reasons:
    ~~~You are under 18 and they want your parent to legally agree
    ~~~Allows them access to your medical history from your personal doc if needed
    ~~~Allows them to share it between SA's and ROTC.

    I would go back an re-read it. My bet is it says you allow fill in the blank (DODMERB) to access your medical records, yadda yadda yadda. Sign here. If under 18 parent sign here.

    Kills 3 birds with 1 stone.

    just wanted to be sure ...

    They only sent instructions re: the medical examination form and that really only pertains to the doctor's part in it all (no instructions re: medical history or extra questionnaire). And although it does, as you mention, state that its re: authorizing others to receive information about my medical records, I just wanted to make sure before sending the package off as it will have to go DHL from over here in Italy and I'd rather send one complete package rather than two.
