Background/Experience Complete

Does anyone know what this form is? It was just added to a couple of the candidates files tonight?
 
It appears that this section was added some time yesterday.

The "Background/Experience Form" was sent after the application online was submitted. It had all of the EC's listed and asked that your Counselor review, edit and comment on all the EC's and add any new ones for Senior year. The counselor had to sign and date and then either scan and email or Fax to Cadet Command.

Which branch of ROTC are you referring to?

what and who would be "your counselor"?. to my knowledge we had no counselor and I know I will sound dumb here, but what is a EC and why would there be new ones for Senior year? thanks
 
Counselor = High School Guidance Counselor
EC = Extra Cirricular (activities such as Student Council, Debate, Community Service, Band, Athletics, Boy Scouts, Civil Air Patrol, etc)

It is simply a verification of what you submit online and has slots/boxes available to check off for confirmed activities you are elected to or validated you are a part of for senior year.

If I remember correctly my DS got this form (requesting hs guidance counselor verify his extra-curriculars) in the mail along with a letter requesting that we have the school nurse or doctor verify his height and weight. We sent them back together once we had gotten both verifications. It was not an online form he filled out, but came via "snail" mail.

Yes it does, thank you. Now I know I didnt forget to fill out a form and I had wondered how did CC know if what someone had checked out was without some stretching of the facts. Not that anyone would think of doing that.

Yep

My DS got this a while back. It was to confirm his senior year activities because he did not list anything he had not actually done yet. His HS principal actually completed it for him.
 
I googled it and found this. But I think the background/experience sheet was the part filled out by the college counselor.

Every applicant gets one of these forms, the applicants high school counselor/advisor checks over the form and signs it and it is sent back to CC. It is the way CC verifies the applicants information.
