Candidate Writing Sample

So I have been working on the Candidate Writing Sample and I was just wondering if anyone had any suggestions on what type of format to use? As in should I be formal about it and include an introductory paragraph, topic sentences, clinchers, etc. or should I answer the questions directly? Any help would be much appreciated!
