CIS - Candidate Activites Record

When recording Activites in CIS, how should I/ what would you prefer or suggest I do to record additional information. (Ex. Girls state ranks, UNIFIED sports, etc.) Would a list with that references the heading of the topic of elaboration work, or should I use a formal paragraph that includes everything?
