confused about Navy medical form???

    The Immunization record form is a no brainer but I am confused about the form titled "naval Health Clinic Annapolis": does our personal health care provide/doctor complete this form after we complete our portion, or do we complete our portion (ie: the questions) and does the health care provider or vaccinator at annapolis complete and sign the back of the form? Am I the only one confused.....LOL! Planning on calling on my day off if nobody knows.

    I was thoroughly confused to! :) I was going to ask on the forums to see if anyone knew and if not also call.

    The Security form is HUGE! :) that was definitely fun filling out :)

    Naval Health Clinic Annapolis

    The front side of the Adult and Pediatric Immunization Questionnaire is to filled out by the patient(plebe) or parents/guardians to provide background info before possible vaccines. The other side is filled out by the Immunization Clinic Staff at USNA. I would wait to sign at the bottom until instuctions have been given. I had to reread this form and in the instruction booklet a couple of times myself. Bring this one along too in hand on I-Day.

    With the front side fillled out you can also show this to your local doctor before any vaccines are given to eliminate possible complications.

    So who does the vaccinations? Do we get these now or wait until Iday?

    We just went in today and got the three it looked like my son was lacking along with a TB skin test. Better to try to get it out of the way now then have VERY sore arms on I-Day!

    My son needs several shots/boosters too ... we are doing it this week. He also needs Hep A, which was not required here when he was a tot ... he'll get one this week, then the follow up at the Academy in 6 months. And I agree ... better sore now than during the summer when there's so much else on their plates!

    Word to the wise--

    photocopy EVERYTHING before you send it in!!

    A. In case it gets lost in the mail
    B. Plebes will need to RE-do that SF-86 (security form) partway through the year. It's helpful if they have those reference's names and addresses handy, instead of having to re-search for them all.

    security packet - "three ppl. who know you"

    With these people, should they be friends your own age or should they be older people? Will the USNA contact these people, and ask for a thorough background, I mean i highly doubt any of the appointees are terroists...

    Make it adults who know your Plebe. None of the people referenced on our Mid's preliminary security questionaire was ever contacted. They are not looking for an "Eyes Only" or "NOFORN" clearance. I assume it is a formality just in case something pops up during a normal background check.

    As above: Make copies of everything!!!

    Definitely copy everything. They ended up losing my acceptance paperwork, and it was a good thing my parents made me make a copy of the signed version and a blank one. It may be tedious to copy all those papers, but it's better to not screw up somewhere along the way and possibly risk your spot.

    Getting back with everyone on the Adult and Pediatric form......I have called the number in the permit to report packet two mornings in a row and no answer, no recording. Perhaps they are only open certain times. At this point I think INZANE99's directions make the most sense so guess we'll proceed accordingly.

    The entire PTR pamphlet can be downloaded via CIS, so there is no need to copy anything.

    Medical Forms

    My father, who is a MD, called per their rquest that only medical providers call for questions. The questionnaire should be filled out to take to your Doc for immunizations, and then have it prepared for I-Day(are you sick, have you had reactions to , etc) Do not fill out back side - that is for I Day and future use. Good Luck getting all done by may 15. I need 4 plus Varicella - ouch!
    Blood will be drawn for Varicella (chickenpox) on I Day and then in a couple weeks all who are not immune will get a booster. So unless you really want the second Varicella now, if you do not have it already, do not get it now - my advice only, per dad through Navy medical. Get MENACTRA asap - it takes 4 weeks for immunity and meningococcal meningitis is a bad actor. Be sure to get at least that and Hep A prior to arrival-dad's advice:biggrin:
