Direct Deposit Form

    Quick question. The Direct Deposit form mentions on it that the financial institution is supposed to mail the completed form to the Academy. But the Instructions for Candidates says to bring completed copies of the form to R-Day.

    So is the bank supposed to mail the completed form, or give it to me to bring on R-Day?

    I know when I enlisted we just completed the form and brought it with us. I would call to make sure but I would just have my bank fill it out and I would bring it with me to R-Day. Hope that helps.

    I filled out two copies of the form. I sent both. The bank rep signed both and returned one to me, saying that I should bring it to R-Day.

    what is the address to send the direct deposit form to USAA?

    Fax number for USAA

    Their fax number is (800) 531-5717. We faxed them the form (after filling out section 1 and 2 per the instructions provided in the Instructions for Candidates Offered Admission. They filled out the rest and faxed in back in 2 days.

    Something that we were told that worked out for us:

    Carry a small folder (you can get them the size of a legal envelope) that you can carry in your hand (or pocket) with copies of your available paperwork.

    Include bank info, direct deposit forms, medical forms, eye forms, social security card, etc. Sometimes paperwork doesn't alway make it where it should, when it should, so if you have these documents handy (in case they have gone missing) it can make life a bit easier, expecially on R day:thumb:

    USAA also has a pre-filled pdf direct deposit form on their website. The only thing needed is for you to fill in your own information and print it out.

    Where can we get the Direct Deposit form at if we do not want to use USAA? I must have overlooked it?

    The box "depositor account title" is in section 3, which says "to be completed by financial institution"

    The Depositor Account Title is the name of the account holder exactly as it appears in the bank records.
