Eye exam tomorrow.
Just checking, but the packet down loaded after completing online paperwork did not include a form for the eye exam. The medical form has a section that is marked SEE OPTO EXAM. But no Optometrist specific form.
Did we miss something?

You did not miss anything! The eye doctor will have forms of their own.

DS completed his DODMERB eye exam in January. The optometrist submitted the results to Concorde electronically. I'm not sure if this is standard practice for all exams.
