DODMERB and reapplication simple form

DD is a re-applicant for class of 2019. The website says that "During your re-application year you will have to complete a simple form verifying your continued health". Where does she find that form? will USNA send that to her in some point in the process?

It's a Present Health Questionnaire. I believe the form is available on the Dodmerb website if you want to be proactive. If I recall from last year, the USNA medical technician emailed it to my DS and asked him to complete it after he got his LOA, but she also told us that if they think your application is competitive they may ask you to fill it out in advance so they have it completed.
 
I'm a reapplicant too and I think you get sent the form after you have submitted 5 documents, or parts, of your application. It's probably set up that way so DoDMERB doesn't waste time & resources on candidates who don't intend on completing their app.
 
I am a reapplicant and received a letter back in September that said that I was medically cleared from dodmerb for the Naval Academy. I believe the continued health form will be sent upon an appointment.
