DoDMERB Fax Cover Sheet?

I need to fax a document to the DoDMERB and I remember seeing somewhere that there is a specific cover sheet. Does anybody know what it is off the top of their heads?
Thanks for any help.
 
The DoDMERB site does not indicate any format for a fax cover sheet.
I would just use a generic cover sheet that contains your name, SSN, candidate number and contact info along with any remedial code that your are referencing.
I would also make sure that each document has your name at the top.

Dodmerb's website states the following:
10. Will you allow me to fax documents pertaining to my medical status to you?

You may fax documents pertaining to your medical status to DoDMERB.

We do ask however, if the total number of pages exceeds 15, that you mail those instead of faxing. DoDMERB has limited ability to receive faxes and faxes exceeding 15 pages could delay our ability to process your documents as quickly as possible.

Please note: If you fax documentation to DoDMERB, you DO NOT need to mail the same document(s). Our fax number is: 719-333-3578. Our mailing address is: DoDMERB, 8034 Edgerton Drive, Suite 132, United States Air Force Academy, CO 80840-2200. DoDMERB has the capability to receive all types of mail deliveries, (i.e., US Postal Service, FedEx, UPS, etc.).

If it wasn't provided in the 'request for additional information' packet - they could care less what the format of the cover is as long as it includes:

To:

From:

Number of pages

Message: (why is this coming to them)

Suggest you (1) put last name and last 4 on each page to help keep it together (2) scan the document, so when they tell you it was lost you can e-mail the scanned copy (why this isn't step one is hard to imagine)
 
If there is a DODMERB identification number, similar to the candidate number you receive for USNA, you should use that in place of any portion of your SSN. DO NOT...DO NOT...send your full SSN on any fax document, that is very bad practice.
