DODMERB Medical Survey Error

When doing my medical survey I accidentally checked Yes for the question, " Have you ever had an X-RAY or any other from of radiation treatment?" Thinking the question was asking if you ever had an X-RAY before. After looking it up I realized it was asking if I have ever had X-RAY treatment, which I have not. I still have not checked the box saying I agree everything is right and it still says my medical survey is incomplete. I can still go back and change any of my No's to a Yes, but not the other way around. What should I do?
 
Does it ask for a description/purpose of the treatment? If so, explain there what it was. I do believe my DD had the same issue and inadvertently checked yes, when if she better understood the question, she would have selected 'No'. She explained it (her confusion) to the doctor during her physical who made the appropriate note and DoDMERB never made it an issue (she did have a remedial for something else).

In short, if you can't change it - submit the form and it will resolve itself later. It really isn't that big a deal. You are committing fraud.

I am all for applicants doing the work themselves, but I do think parents should help while completing the DoDMERB process.

(I just got back from DD's Eye Exam and saw the OP's Question.) My DD just filled her DoDMERB out last night. I sat next to her as she did it for most of it. That question required parsing !

I will add-on to Cerberi's advice.

Go back into your Portal to the FAQ section and it states that you must use BLACK PEN TO CROSS out the Error (READ the Instructions !) on the Print out of the form that you will take with you for the Exams.

In addition, because you checked the "Yes" Box by accident, it will add an "explanation" portion to the DoDMERB Form. In each of the Boxes that require Explanation state:

" My Answer was in Error, I have NOT received X-RAY or any other from of radiation treatment" If the page will not allow you to leave out the Date, put in today's date.

BTW, Fill out the Form using a Fire Fox Browser to avoid the various glitches you will get in Explorer and Chrome. Not doing Fire Fox cost DD and me an hour of our lives.
