Employer's Evaluation of Candidate

I read on my portal that the Employer's Evaluation of Candidate Form should be filled out if a candidate works during the week. I do work during the week, but on my portal everything is green. There is no red mark for the Employer's Evaluation of Candidate Form, and I have not been notified by my FFR that it is missing from my profile. Should I submit this form?
 
There's no mark on the portal indicating if it's complete. You just download it off the portal and send it in, then admissions will add it to your file.
 
I was confused by this too - I sent it in, was told by my FFR that they didn't have it, and sent it in again. Hope they have it now, because she hasn't been able to tell me. Apparently, you only get credit for working added to your application if they have an Employer's Evaluation - and, of course, they say nice things about you (I have no idea if my boss did that, but oh well XD).
 
If you get any good recognition (School, community, church, club etc) then send it in to your RC. Although it might not be a part of the official information required, it might increase your Whole Person Score and increase your chances of admission.
