You know, when stuff like this (well, its equivalent) happens in the military or in industry, the outcome is a thorough review of policies and procedures to prevent such an error from happening again.

I HOPE that's happening in the intelligence community, but for some reason, I don't think it is.

Oh well. Time will tell, I suppose....
