Last Name Question

Hello, my DD and I are finishing up her appointee kit to USAFA. She has two last names, but prefers to be referred by her first. Does anyone know what we should do to address this preference? Thank you .

I don't know the answer, but I think many would recommend your daughter use the last name that was provided by a parent or guardian at or near the time of her birth. Typically, there are future background check and security clearance considerations to contemplate in providing a name other than what was previously provided to the government. In the past, the name provided by those seeking military service was the name on the certificate of birth, absent a subsequent legal name change. The reason I suggest providing one's legal name is because adopting name preferences that contradict legal documents may present immediate application hurdles or future inquiry. However, you may want to ask U.S. Air Force Academy Admissions.

What’s on her most recent driver’s license or more importantly her Passport?

I would think this would be included in instructions? I know USNA has very detailed instructions as to how to fill out each item.
