DS' father and I were never married. Bio father is listed on his birth certificate, but we haven't seen/spoken to him in 5+ years. I don't have a good address or phone number or email for him, and I'm not sure what he's doing. He has no custody of DS (and never has), but technically hasn't terminated his parental rights (based upon our state laws).
My DH is listed as DS' father in virtually all records - school, medical, CAP, Scouts, etc. He would be listed as DS' other parent on his FAFSA.
Whom should DS provide information for on his NROTC application?
